A young man born with a unilateral cleft lip and palate deformity underwent corrective surgery as an infant. However, the passage of time led to a nasal deformity, nasal asymmetry, and a prominent lip scar. These concerns, along with the upcoming start of his post-graduation journey, prompted him to seek further corrective surgery. Choosing the Right Treatment After extensive research, the patient and his parents chose our hospital for his corrective surgery. During the initial consultation, a comprehensive examination revealed the missing lateral incisor, lip scar, and nasal deformity. The patient’s desire to address all these issues was carefully considered when formulating the treatment plan. Expert Treatment Plan for Optimal Results Due to the patient’s desire for minimal scarring, a closed rhinoplasty was chosen. This technique ensures zero visible scarring by making all incisions inside the nasal cavity. Additionally, the treatment plan included: Rib bone and costal graft harvesting: These grafts were used for alveolar grafting and nasal augmentation, respectively. Dental implant placement: A dental implant was placed at the missing lateral incisor site, followed by bone grafting to promote osseointegration and bone graft consolidation. Closed rhinoplasty: The lower crus of the lateral cartilage was excised, followed by tunnelling of the costal graft to augment the nasal bridge. This created a sharper nasal profile. Lip scar revision: Excess scar tissue from the cleft lip surgery was excised, and the incision was closed with fine sutures. Alar base narrowing: This final procedure addressed the patient’s concern with the broad alar base. Broken Front Tooth Removal and Dental Implant Surgery

He is an avid sportsman and has been involved in sports from a young age. Around five years ago, while playing football, an opponent’s head had crashed against his upper left central and lateral incisor teeth. Since it had been around the time of his tenth board exams, he had self-medicated with analgesics without visiting the dentist and the pain had subsided. A few months later, he noticed that the crowns of both teeth had turned slightly darker. There was also a dull ache in the region. He had rushed to his family dentist who had informed him that the two incisors had turned nonvital due to the injury. Nonvital tooth root canal treatment successfully completed on the two upper incisors Root canal therapy had been advised for the two teeth and had been successfully completed. The pain subsided completely. Ceramic crowns had been advised by the dentist for the two teeth. The patient had however completely forgotten about that and had got on with the business of daily living. Reinjury to the same two teeth while playing competitive cricket with friends A few days ago, the patient had been playing cricket with his friends. There was a repeat of the previous accident when he collided with another fielder while trying to complete a difficult catch. This had ended badly for him with the lower half of his lateral incisor cleanly breaking off. There was a lot of pain in the region following the collision. It was only after he got back home that he noticed that the central incisor was mobile. His parents immediately rushed him to their dentist. He obtained radiographic images of the two teeth and explained to them that there was a horizontal root fracture of his central incisor. Explaining further, he said that the tooth had to be extracted. Realizing that the tooth would be lost, the patient wanted a dental implant to replace the missing tooth. Necessity for atraumatic extraction of fractured tooth fragments for implant success The dentist explained to them that the broken root fragment had to be removed atraumatically to ensure that the socket structure was preserved. This would have to be done very skillfully as a root canal treated tooth would be brittle and prone to crumbling. He said that this was imperative for successful dental implant surgery. The American Association of Endodontists advocates this treatment protocol for horizontal fractures of anterior teeth. Artificial teeth in the form of porcelain crowns are fitted onto the implants. Gum tissue modification or bone grafting is not necessary per this protocol. There are also other types of crowns including zirconium used for this treatment. The dentist had informed parents that they needed to get this done at a specialty dental implant hospital. He had then referred the patient to us for further management. Her lacerated gums and lips were treated at a local dental clinic nearby her residence. The young patient feels very odd and distressed to be without teeth. She grieved about her friends teasing her at school with funny names for her unpleasant smile without teeth. The patient’s parent demanded for a fixed option to replace her missing upper front teeth. [/vc_column_text][vu_heading style=”2″ heading=”EXAMINATION OF THE PATIENT ” subheading=”” alignment=”left” custom_colors=”” class=””][vc_column_text]On clinical examination, there was missing teeth in the upper front region. The surrounding gum tissues were fine with no pathological findings. On radiological examination, OPG taken shows complete missing of upper anteriors with no remnants of teeth inside the bone. No abnormalities seen in the surrounding bone structure. Over the time there was an increase in the mobility of the adjacent teeth and the dental implant. The anterior teeth and the implant exhibited mobility. The surrounding gum tissues were red and swollen. Full mouth X-ray (OPG) taken shows apicoectomy done to the root canal treated teeth in the lower anteriors. Infection around the implant and the teeth was evident. The patient stated that she’s been experiencing severe mobility of her lower front teeth for a while as a result of which one of her lower anterior had fallen out recently. Since then, the gaps between her lower front teeth have started to get wider and the persistent mobility of her teeth often gave her tough time pronouncing certain words. The unpleasant look of her lower teeth worried her. She requested for a solution to bridge the gap between her teeth in order to restore the normalcy of her oral function and appearance. Patient gives a medical history of hypothyroidism, diabetes for the past 10years and is known to be on medication.[/vc_column_text][vu_heading style=”2″ heading=”EXAMINATION OF THE PATIENT” subheading=”” alignment=”left” custom_colors=”” class=””][vc_column_text]On clinical examination, the lower anteriors exhibited severe mobility with missing right central incisor. A course of periodontal therapy (gum treatment) was also intended, as the supporting gum tissues were weak due to underlying periodontal pathology. Hence the final impression was taken, followed by few trials of bite alterations of the final prosthesis. A natural looking ceramic bridge was fixed onto the dental implants covering the unpleasant gap which was worrying the patient.
