Ectopic Eruption – Maxillary Sinus Impaction surgery
Patient develops discomfort and heaviness in right side of face The patient is a 24-year-old male from Chennai in Tamil Nadu, India. He had developed a mild pain and a slight swelling in the right side of his upper jaw. This has been ongoing for the last 7-10 days. He has a history of sinusitis for the past few years. The swelling had become noticeable and he had made enquiries regarding the best hospital to get this treated. He had been referred to our hospital by his family doctor. Initial examination at our hospital Dr SM Balaji, Oral and Maxillofacial Surgeon, examined the patient and obtained a detailed history. A 3D CT scan was obtained to fully visualize the maxillary sinus. This revealed that the sinus was filled with exudate. There was also an impacted third molar, which was found embedded in the floor of the orbit within the sinus cavity. This was diagnosed as an ectopic third molar. Removal of such ectopically impacted teeth has to be performed with utmost care due to its proximity to the orbital floor. Training for simple impactions are imparted to postgraduate students in dental schools. Complicated impactions such as this need to be performed by experienced oral and maxillofacial surgeons. It was explained to the patient and his parents that this needed to be removed surgically. The contents of the sinus cavity would also be cleared during surgery. They were in full agreement with the treatment plan and consented to surgery. Caldwell Luc procedure is the technique of choice for such impactions. Our hospital is a specialty center for Oral and Maxillofacial Surgery. Jaw joint surgery, orthognathic surgery, surgery for removal of odontogenic cysts and facial trauma surgery. Presence of an ectopic tooth and its implications Ectopic tooth eruption in the maxillary sinus is usually an incidental finding that is discovered during routine radiological examination. They could also become symptomatic and cause discomfort and pain as in the case of this patient. Development of a dentigerous cyst in relation to an ectopically erupted maxillary third molar within the maxillary sinus is very rare. These impactions can also arise from displaced dental follicles. Ectopic eruption occurs when a tooth is displaced from its normal position. This could be from pressure caused by pathological conditions such as cysts. It can also be idiopathic with displacement of the tooth bud occurring during development. This results in its eruption at an abnormal site. Such ectopic eruptions are most commonly seen in the dentate region. Successful removal of ectopically erupted right maxillary third molar tooth Under general anesthesia, a crevicular incision was made in the right maxillary region. A flap was then elevated to expose the maxillary bone. A window was made in the maxillary bone through the Caldwell Luc technique to gain entry into the maxillary sinus. This was followed by thorough sinus clearance. The impacted right third molar was removed. This was followed by irrigation of the sinus with antibiotics and closure of the wound with sutures. Complete resolution of symptoms after maxillary molar impaction surgery There was complete resolution of the patient’s swelling and discomfort at the time of checkup. The patient and his parents expressed their satisfaction before final discharge from the hospital. Surgery video
Cheekbone fracture surgery and lower eyelid correction
Patient with facial injuries This is a 35-year-old patient from Jharkhand. He was hit by a speeding car which resulted in facial trauma before 2 years. The facial trauma resulted in facial bone fractures. He underwent emergency treatment for cheekbone fracture and lower eyelid correction in Jharkhand. The patient was not happy with the outcome of cheekbone fracture surgery. He complained of depressed cheekbone in the left side of the face. He requested for further cheekbone correction. Ectropion of eye He also complained of double vision in one eye (left) and lower eyelid drooping. His lower eyelid was sagging outwards thereby exposing the surface of the inner eyelid. There was noticeable difficulty in closing his left eye. He requested left lower eyelid surgery. The patient’s eye doctor had neglected the need for a second surgery. He advised him to use eye drops every day to prevent drying up of the left eye. Cheekbone fracture surgery in India Complete clinical and radiological evaluation done. Oral and Maxillofacial surgeon Dr. SM Balaji diagnosed malunited cheekbone fracture. He had hypoglobus and ectropion of the left eye. There was also a left orbital floor fracture which led to herniation of the orbital contents. Fracture treatment along with ectropion correction Incision placed through the previous surgical scar. Layers dissected, thereby exposing the previously placed plates and screws. Removal of plates and screws done. The malunited cheekbone fracture was re-fractured. Re-fractured segment elevated and fixed using plates and screws. The floor of orbit reached. The herniated orbital floor contents released. Left orbital floor reconstruction surgery done using Titan Medpor implant. The implant was fixed using screws. Malpositioned lower eyelid released from the scar tissue. The ectropion of the left eye was also corrected using a medial canthal incision. Surgical outcome The check bone fracture corrected. The lower eyelid raised and reattached to its normal position. The orbital floor was also reconstructed successfully. The patient was happy with the outcome of the surgery.

Dr SM Balaji at the University of Buenos Aires Hospital’s Department of Oral Surgery Dr SM Balaji was invited by Prof Sergio Gotta, Head of the Department of Oral and Maxillofacial Surgery, University of Buenos Aires. He was given a guided tour of the Department of Oral and Maxillofacial Surgery by Prof Gotta accompanied by Dr Maricel Marini, Oral and Maxillofacial Surgeon, Department of Oral and Maxillofacial surgery and residents of the department. Prof Gotta explained to him the various measures that had been put in place for the benefit of the patients. Dr SM Balaji interacts with the staff of the hospital Dr SM Balaji spent some light hearted moments at the hospital cafeteria with Prof Gotta, Dr Marini and the residents. He later gave them complimentary copies of his Clinical Cranio Maxillofacial Surgery textbook and the current issue of the Annals of Maxillofacial Surgery, a Scopus indexed journal for which he is the Editor in Chief.
Azerbaijan Society of Oral and Maxillofacial Surgeons Presentation Ceremony
Dr SM Balaji invited to attend the presentation ceremony of the Azerbaijan Society of Oral and Maxillofacial Surgeons (AzSOMS) Dr. SM Balaji was invited to the presentation ceremony of the AzSOMS. This was held at the Oral and Maxillofacial Surgical Clinic of the Azerbaijan Medical University in Baku. Prof. Chingiz Rahimov, President, AzSOMS, had extended this invitation to Dr. SM Balaji. This was in recognition of his services and contributions towards craniomaxillofacial surgery. Dr SM Balaji introduces the International Cleft Lip and Palate Foundation to the audience As the authorized representative of the International Cleft Lip and Palate Foundation (ICPF), Dr. SM Balaji introduced the ICPF to the audience. He spoke about his passion towards the rehabilitation of children born with cleft lip and palate. Dr. SM Balaji also spoke about his extensive involvement with the International Cleft Lip and Palate Foundation. He described how the organization has touched the lives of patients with cleft lip and palate through its humanitarian surgical missions to poor nations. Dr. SM Balaji’s keynote speech on the Management of Craniofacial Clefts Dr. SM Balaji delivered the keynote speech on the topic of the Management of Craniofacial Clefts. He described key cases from his over 30 years of experience performing craniofacial surgeries. His keynote speech documented innovations made by him that led to significant improvement in the quality of life of the patients. The interactive Q&A session at the end of his keynote speech was very lively. Audience members asked him many questions on the management of craniofacial clefts. Prof SM Balaji visited the Department of Oral and Maxillofacial Surgery, Stroger Hospital of Cook County, Chicago at the invitation of the Chairman of the department, Dr Henry C Fung as an observer.
